Three check items for anemia diagnosis, part 1: serum ferritin (SF)

1. What is anemia?

Anemia refers to a common clinical symptom in which the volume of red blood cells in the peripheral blood of the human body is reduced and lower than the limit of the normal range.

2. Who is prone to anemia?

kid

①Infants and young children: The incidence of anemia is relatively high in babies between 6 months and 2 years old.

②Children: Children under 12 years old need extra iron as they grow. Children who are picky eaters will increase the risk of anemia.

female

Women with irregular menstruation and heavy menstrual flow are prone to anemia if they have uterine fibroids or adenomyosis. Pregnant women and growing fetuses need more demand for iron. If they do not increase their iron intake, they are prone to iron deficiency anemia.

Middle-aged and elderly people

Chronic blood loss in the digestive tract can lead to anemia. Gastrointestinal tumors are relatively common in middle-aged and elderly people, such as gastric cancer and intestinal cancer, so they need to be vigilant.

3. What are the symptoms of anemia?

Weak resistance, often feel sick, dizzy; looks pale, memory loss, poor learning ability; unable to keep up with physical strength; drowsy and unable to wake up.

4. Three tests for anemia

Clinically, bone marrow morphological examination is often used for the differential diagnosis of anemia. However, this method is invasive and the test results are easily affected, so its application has certain limitations. In recent years, three serological indicators of anemia have been widely used in the diagnosis of anemia, including serum ferritin (SF), folic acid (FA), and vitamin B12 (VB12) detection.

(1) Serum ferritin (SF)

Serum ferritin (SF) is an iron-containing protein complex that is mainly synthesized in the liver and stored in the spleen, bones, and other tissues. It reflects the total amount of iron stored in the liver and the nutritional status of the body and is an effective indicator for detecting iron deficiency and iron overload diseases.

Clinical significance:

1. Differential diagnosis of anemia: In iron deficiency anemia, serum ferritin (SF) would be significantly reduced. When SF is less than 12ng/ml, it is considered an indicator of iron storage and consumption in the body, and when it is less than 50ng/ml, it indicates the possibility of iron deficiency. While SF would be normal or elevated in aplastic anemia and thalassemia, .

2. Chronic diseases: Such as anemia caused by chronic infection, inflammation, connective tissue diseases, malignant tumors, etc. Abnormal iron metabolism manifests as decreased serum iron, normal or increased ferritin.

3. SF levels may increase in malignant hematological diseases such as aplastic anemia, acute and chronic leukemia, and malignant lymphoma.

4. Ferritin can be used as a tumor marker. For example, when suffering from liver cancer, lung cancer, pancreas cancer, leukemia, etc., the ferritin synthesized by cancer cells increases, causing the serum ferritin to rise.
