Targeted Monitoring Value of Fecal Calprotectin in the Prognosis of Ulcerative Colitis

Ulcerative Colitis (UC) is a chronic inflammatory bowel disease that affects the colon and rectum. It is characterized by recurring episodes of inflammation, leading to symptoms such as abdominal pain, diarrhea, and rectal bleeding. Earlier detection and monitoring of UC are crucial for effective management and improved patient outcomes.


A recent study has shed light on the targeted monitoring value of fecal calprotectin, a calcium-binding protein found in the stool, in predicting the prognosis of UC. Studies have shown that although FCP had only modest cross-sectional accuracy in identifying disease activity, an FCP concentration of ≤250 ug/g vs.>250 ug/g was associated with an increased probability of achieving long-term clinical, endoscopic, and histologic remission, and reduced probability of colectomy and UC-related hospitalization. FCP concentration may be a valuable predictor of long-term outcomes in biologic-treated patients and is a potential surrogate for histologic measures of disease activity within a treat-to-target paradigm. FCP concentration can be more easily measured compare to endoscopic and histological detection, which may make it a suitable marker for early monitoring.
